About the Role
This position reports directly to the RD Coaching Team Lead and is responsible for a roster of Level 2 diabetic members who sign up for coaching. The estimated roster sized is 175 active participants, but this amount may vary based on business needs. This position is the primary and key relationship with members that helps each member focus on behavior change through lifestyle changes. The RD Lifestyle Coach is responsible for each individual member, including connecting the member with other Level 2 team mates as needed, such as, Nurses, support, etc. Areas of responsibility include coaching skills, behavior change focus, understanding diabetes and reversal/remission, educating on nutritional interventions for diabetes, continual training, motivating and encouraging members, achieving outcome, member focus and contributing to the accomplishment of the Level 2 program and coaching operations.
An RD Lifestyle Coach is responsible for conducting RD referrals. Referrals are made from Lifestyle coaches and clinical staff for in-depth nutrition education around diabetes, but also other disease states or nutrition needs. Referrals require a blend of lifestyle coaching and nutrition expertise education to help promote behavior change. RD Coaches must understand how to read and react to CGM data, disease state information, biometrics, and other data that may be available to understand a member's health conditions.
An RD Lifestyle Coach provides telephonic and video coaching for diabetic members desiring to reverse the effects of diabetes using CGMs (Continuous Glucose Monitors), fit bits, specialized apps and other resources to bring awareness and promote change. Level 2 RD Coaches partner with participants and co-managing nurses and Lifestyle Coaches to establish a personal wellness plan focusing on nutrition, physical activity, mindfulness and medication remediation through our clinical team. During a typical call, the RD Coach will listen to the consumer's needs and work with them to develop and document a realistic and evidence-based wellness plan in order to help the consumer accomplish their wellness goals. The RD coach will personalize the program to the consumer to activate outcomes progression. RD Coaches are supported by online curriculum, call frameworks and skill enhancement tools.
The RD Coach will meet with member based on member need and risk related priority levels. The Coach will review such things as CGM data, exercise data, nutrition information, etc. in order to support the member in moderating their glucose levels and ultimately de-escalate from medications when appropriate. The RD Coach will support each member in areas of change to ensure they have key tools, information, technology and coaching support to achieve their goals.
RD Coaches will have the responsibility to clearly understand the coaching process and behavior change, an in-depth understanding of nutritional needs not only with diabetes, but other disease states that affect cardiac, nephrotic, renal, and digestive function, among others. The RD Coach must understand the tenants of Level 2 to achieve remission, and review management of members and ultimately success of the program. RD Coaches will also be responsible for group coaching on various nutrition topics that relate to diabetes management.
There may be projects and/or pilots the RD Coach will be involved in depending on business needs. RD’s will develop nutritional trainings and deliver these to L2 coaches and staff as needed. RD’s will participate in developing reference materials for coaches to use to help educate members on nutrition topics.
RD’s are expected to maintain their RD licensure during employment with L2. On the job training is required and essential for coaching success.
You will have the opportunity to attend NBHWC training during business hours in order to obtain board certification upon the start of employment with Level2. You will be required to obtain certification within a timeframe that aligns with the testing calendar dictated by the NBHWC.

- Ability to empathize, motivate, and encourage all members towards healthy changes
- Conduct group coaching sessions that focus on education and lifestyle changes through coaching behavior change models
