This position requires travel and onsite services to the locations noted. The Mental Health Clinician serves as a key member of the interdisciplinary care team and provides behavioral health assessments, care planning, therapy services, behavioral intervention, and case management to the independent living population within the designated region. Requirements:

This position requires travel and onsite services to the locations noted. The Mental Health Clinician serves as a key member of the interdisciplinary care team and provides behavioral health assessments, care planning, therapy services, behavioral intervention, and case management to the independent living population within the designated region.
The ideal candidate will meet the following requirements:
- Masters or doctoral degree in psychology and/or social work with specialized expertise in geriatrics
- Board certification in geriatrics and expertise in serving people living with dementia is preferred
- Minimum of three years' experience as a licensed behavioral health clinician providing assessment and psychotherapy services
- Maintains a state license as a mental health services provider, i.e. Licensed Clinical Social Worker, Licensed Psychologist or Licensed Professional Counselor
- Eligible for National Provider Identification number AND credentialing as a behavioral health clinician by Medicare and other third-party payors
