Job description

We are looking for a Registered Nurse to admit patients and be a case manager, with strength in hospice care to manage the wellbeing of a full case load census. You will be responsible for educating them and their families on hospice benefits. The ideal candidate will be caring, kind, empathetic, and well-trained to be able to give competent nursing care with little supervision. You will be compassionate and cool-tempered. You will be able to follow guidelines per the company and per Medicare. Our goal is to promote patient’s well-being by providing high-quality end-of-life care.

Pay will be per visit plus mileage. Admissions are a separate rate. Rates depend on your experience and qualifications.

Responsibilities

• Be well versed and educated in what hospice is and be able to communicate with patients and families.
• Assess patient needs and address in visit as necessary
• Observe and interpret patient’s symptoms and communicate them to physicians
• Collaborate with physicians and nurses to devise individualized care plans for patients
• Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patients’ charts
• Adjust and administer patient’s medication and provide treatments according to physician’s orders
• Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.)
• Provide instant medical care in emergencies
• Supervise and train LPNs and nursing assistants
• Foster a supportive and compassionate environment to care for patients and their families
• Expand knowledge and capabilities by attending educational workshops, conferences etc.

Skills

• Proven experience as a registered nurse
• Excellent knowledge of nursing care methods and procedures
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
• A team player with excellent communication and interpersonal skills
• Responsible, compassionate, competent
• Outstanding organizational and multi-tasking skills
• Patient with excellent problem-solving skills
• BSc or other diploma from a nursing program
• Valid nursing license

Pay: $65.00 per hour

Weekly schedule:

• Monday to Friday
• Weekend availability

Education:

• Associate (Preferred)

Experience:

• Nursing: 1 year (Preferred)
• Case management: 1 year (Preferred)
• Hospice care: 1 year (Required)

License/Certification:

• BLS Certification (Preferred)
• RN (Preferred)

Willingness to travel:

• 100% (Preferred)
