Job Summary for Registered Nurse:

• The RN Performs home care visits after the initial assessment, collects on going data, and reports to the RN Clinical Supervisor, implementing the POC.
• The RN Provides skilled nursing care in the patients home as ordered by the attending physician in conjunction with the Plan of Care.
• The RN Submits clear and appropriate documentation in a timely manner and documents patient education.
• The RN Communicates effectively to physicians, supervisors, and co-workers in a timely manner.
• The RN Promotes continuity of care with appropriate admissions, transfers, and charges.
• Other duties as assigned.

Skills:

• Effective communication skills (verbal and written)
• Experience with Electronic Medical Records (EMR) software is preferred
• Time management and Organizational skills
• Demonstrated ability to work autonomously
• Excellent planning, organizing and communicating.
• Prior Nursing experience in Hospice or Home Health setting preferred.

RN Position Requirements:

• Current Ohio RN License
• BLS/CPR Certification
• Local travel is required; must have a reliable vehicle with proof of auto insurance and valid Driver's License
