AURORA Home Health is seeking an experienced Home Health Registered Nurse for field visits in Fort Worth, TX area. Ideal candidate will have a year or more of recent home health experience. Must have clinical assessment skills and knowledge of OASIS D1.

About the Position:
Registered Nurse are responsible for comprehensive assessments in the patient's home, including the completion of OASIS using electronic documentation (Axxess).
- Evaluate patient and home health environment to determine required services and involve the patient and patient's family in developing the plan of care.
- Develop the POC based on findings from evaluation visit.
- Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care.
- Administer medications and treatments as prescribed by physician
- Provide direct supervision of nursing clinical care team members (LVN's & HHA's).
- Participate in regular case conference and in-services
Requirements
- One year of previous adult home health experience required
- Current Texas Registered Nurse (RN) license
- One year of clinical nursing experience
- Current Texas driver's license
- CPR Required
- Experience with healthcare software, Axxess is a plus
- One year of previous OASIS experience

Pay: $75,000.00 - $80,000.00 per year
