Your Choice Health Care, LLC is currently looking for a RN Case manager to join our team for the Gallia, Meigs and Jackson County areas

What we are looking for:

• Graduate of an approved school of professional nursing.
• license to practice nursing in state of Ohio.
• Current drivers license
• pass a BCI background check and drug screen.
• Good written and verbal communication skills

What you will be doing:

• Provide care and treatment in the patients home.
• Some of the tasks required: wound care, IV therapy, disease teaching and management, telehealth, medication education and management, venipuncture skills, foley catheter care, assessments and care planning process. Electronic documentation.
• Act as liaison for patient physician and external case managers
• Supervisory visits for paraprofessional staff

Job Types: Full-time, Part-time, PRN

Pay: $57,000.00 - $72,000.00 per year

• Gallipolis, OH 45631: Reliably commute or planning to relocate before starting work (Required)

Experience:

• Nursing: 1 year (Preferred)

License/Certification:

• BLS Certification (Preferred)
• RN (Preferred)
