Job description

The Registered Pharmacist fills orders for parenteral medication when needed; monitors patient drug therapies; and provides drug information. Compounds and dispenses prescribed drugs in a healthcare facility environment. The Registered Pharmacist is responsible for transferring prescription; handling incoming physician calls; logging data into the computer; and drug utilization review.
Qualifications
- Bachelor's degree in Pharmacology or related field from an accredited institution
- Pharm D
- Registered Pharmacist in the state of Illinois
- A minimum of one (1) year of experience as a Registered Pharmacist in a hospital setting
- Experience in unit-dose, IVs, hyperalimentation, and chemotherapy
- Experience with Microsoft Office computer programs
Responsibility Highlights
- Prepare and dispense medication orders per physician’s request according to established policies, procedures and protocols.
- Interpret medication orders.
- Maintain accurate, complete patient medication profiles.
- Compound and dispense pharmaceuticals including sterile chemotherapy and parenteral nutrition products accurately.
- Ensure safe, appropriate cost effective medication therapies for patients according to established policies, procedures, and protocols.
- Monitor drug therapy regimens for contra-indications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and use.
- Detect and report suspected ADRs accurately and in a timely manner.
- Sustain formulary by minimizing non-formulary drugs, using therapeutic substitution and promoting rational drug therapy selection.
- Suggest appropriate, cost effective therapeutic alternatives to medical staff.
- Provide accurate, adequate, and timely drug information to professional staff and patients and their families (if needed).
- Document all clinical activities and interventions accurately and completely.
- Participate in the quality improvement and medication use review activities of the department.
- Collect data; conduct quality monitors and inspections; and maintain logs, records, and other documentation as assigned.
- Contribute to quality and effective operation of the department.
- Supervise and direct Pharmacy Technicians.
- Verify the daily activities of Pharmacy Technicians.
