We are looking for motivated, goals-driven and dynamic individuals to monitor clients in our Chronic Care Management and Remote Patient Monitoring Programs.

Duties:

Answer client calls and questions to inquire of CCM/RPM services

Determine client eligibility for CCM/RPM services

Contact clients who are enrolled in the program to coordinate care with their physician and health coach

Work in partnership and collaboration with healthcare team

Schedule client appointments with their health coach

Monitor glucose levels, blood pressures, weight, and oxygen status of enrolled clients and report abnormal results to the client's health coach

Document in the electronic medical record and CCM/RPM software systems

Other duties as assigned.

Qualifications:

Medical office experience

Communication skills and confidence when speaking with clients and healthcare team members.

Proficient computer skills. Able to accurately document in an electronic medical record and CCM/RPM software system.

A team player who can follow specific workflow processes to achieve goals

Able to maintain confidentiality and comply with all applicable laws regarding patient information and privacy acts

Requirements:

Computer with high speed wireless internet

Safe and secure work environment to ensure client privacy

Subject to background investigation and/or fingerprint check in addition to random drug screenings

Job Types: Full-time, Part-time

Pay: $14.00 - $16.00 per hour

Schedule:

• 8 hour shift

Experience:

• Medical Office: 1 year (Required)
• CCM/RPM monitoring: 1 year (Preferred)

License/Certification:

• Certified Medical Assistant Certification (Preferred)
