Star Kids Pediatric Home Health has an opening for a part time case manager. 15-20 hours/week. Great office loation in American Fork -close to VASA fitness! Qualified candidates must have working vehicle and clean driving record. Mileage is reimbursed.

Overview:

Star Kids Pediatric Home Health is a great place to work with friendly office and field staff. The RN Case Manager is responsible for analyzing and researching patient recertification needs, participate in case conference, and offer educated opinions to the team. A great candidate for this position will have a strong understanding of good nursing practice and the ability to identify issues and ensure appropriate resolutions to issues by building relationships with assigned pediatric patients and caregivers. The candidate must be able to publicly represent Star Kids' mission and vision for this special population at all times. Understanding of private duty nursing is preferred, but not required. However, home health or acute care case management experience is required. A Utah registered nursing license is required for all candidates. Responsibilities:

· Submits recertification paperwork on time and within CMS requirements.

· Visits patient families in their home to determine qualifying certifications required by CMS.

· Works with providers and families to create and manage each patient plan of care.

· Identifies quality, infection control, and nursing needs trends regarding assigned patient roster.

· The right candidate will communicate their recommendation for care/needs to the supervisor/manager.

· Highly organized candidates will be able to manage scheduling issues and resolve time conflicts quickly so as to be compliant with CMS home health requirements.

· Follow-up (phone, e-mail, and regular mail) with caregivers, providers, and management regarding patient needs, changes to plan of care, or scheduling conflicts.

· Communication with billing department on a daily/weekly basis to assist with authorizations and appeal letters using the established insurance guidelines to resolve collection barriers.

· Reports departmental issues and concerns directly to case management supervisor.

· Utilizes systems, various documents, and reports to identify errors accurately and within established timeframes.

· Meets productivity and accuracy expectations of the position

· Other duties as assigned that may likely include office filing, scanning, and administrative work, as needed.

Minimum Qualifications:

· Clean Utah Registered Nurse license.

· Clean driving record with well-maintained vehicle.

· One year of home health or acute care case management experience with thorough knowledge of authorization process required.

· Availibility to manage time according to the needs of family caregivers and job demands. The office is open Mon-Thurs 9-4, Friday 9-3. The right candidate may occasionaly have to work outside of these hours to accomidate patient's schedules (this is a rarity, but should be mentioned).

· Strong ability to empathize with people, problem solving skills, and communication abilities. We LOVE a great attitude and critical thinking skills!

· Intermediate skills in Google Sheets, Microsoft Excel, and Word required.

· Effective verbal and written communication skills required.

· Applicants must demonstrate the potential ability to perform the essential functions of the job as outlined in the position requirements through a 90 day probationary period.

Physical Requirements and Working Conditions:

· Mostly sedentary work, requiring lifting up to 10lbs

· Repetitive motion associated with operating a computer and other office equipment

· Inside, climate-controlled working conditions. Some driving is expected.

Schedule: Monday to Friday

Education: Associate of Science in Nursing or higher.

Experience: Case Management: 1 year or more (preferred experience in home health or acute care setting)

Benefit Conditions: 1st day of the month following 30 days after hire. This is available to all staff working greater than 30 hours/week.
