Extended Family Care has IMMEDIATE OPENINGS for PART TIME RN Clinical Supervisors 16-24 hours per week (8-hour shifts). This is a perfect opportunity for RN’s who are interested in remaining in the nursing profession but need a less demanding work schedule due to personal obligations.

Our valued part-time RN Clinical Supervisors are responsible for their own caseloads that they manage clinically with the support of other team members. This approach allows our clinicians to get to know their patients better, allowing for continuity of care for patients and staff alike.

SCHEDULE

• 2–3-day shifts (8 hours shifts) on weekdays

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE, but not limited to:

• Independently implements the nursing process for patients and families.
• Obtains patient’s medical history, particularly as it relates to the patient’s present condition, from the patient and/or family member.
• Performs in person physical assessments according to visit schedule to identify patient needs.
• Evaluations of the patient’s family dynamics, home situation and environment to ensure appropriate support is in place.
• Establishes, maintains and/or revises the patient’s plan of care.
• Evaluates the effectiveness of the plan and makes revisions based on patient’s changing needs.
• Informs physician of changes in the patient’s condition, plan of care and/or frequency and type of services needed.
• Consults and coordinates care with other active caregivers.
• Supervises personnel assigned to provide direct care to the patient.
• Orients and provides additional training as needed to field staff to ensure care is being provided in accordance with the plan of care.
• Submits clinical and supervisory documentation to the branch according to requirements.

JOB REQUIREMENTS

• Graduate of an approved school of nursing. Must possess a current license to practice in Pennsylvania as a RN. Minimum of 3 year’s experience as a practicing Registered Nurse.
• Previous home health care experience a plus but not required.
• Must be able to read and interpret documents and manuals. Ability to complete electronic documentation. Ability to speak effectively with patients, families, and employees.
• Must have reliable transportation – individual may make multiple home visits on any given day based on caseload.
