This position supports the Mission, Values, and Vision of Carolina Family Health Centers, Inc. Under the direction of the primary care provider, the Licensed Practical Nurse is responsible for providing medical case management services to a specific panel of patients within the clinical setting. This position will also perform Medicare Annual Wellness Visits (AWV). This position reports directly to the Population Health Manager.

Case managers work to facilitate patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress. Oftentimes case managers work with physicians, social workers, families, and human services providers. The overall goal for case managers is to improve clinical outcomes, increase patient satisfaction, and promote cost-effectiveness.

Minimum Qualifications:

• Current Licensed Practical Nurse license.
• BLS Certification.
• Previous experience in a community health center setting in the area of nursing and/or case management is preferred.
• Ability to travel 50% of the time to either Harvest Family Health Center (Elm City, NC) or Freedom Hill Community Health Center (Tarboro, NC).
• Active North Carolina Driver License.
• Effective communication and interpersonal skills.
• Must demonstrate the skills and judgment necessary to provide direct care to patients and nursing interventions under the direct supervision of providers.
• Strong computer skills and working knowledge of the windows-based environment.
• Must have experience with electronic health record.
• Bilingual in Spanish, and previous pediatric case management experience preferred but not required.

Physical Requirements:

• Frequent sitting, occasional bending, stooping, lifting, reaching forward and overhead.
• Almost constant standing and walking.
• May occasionally be required to lift patients, office equipment and paper, usually not exceeding fifty pounds of direct lift.
• Frequent computer work required.
• Must be able to travel freely in the local area.

As a Medicaid/Medicare facility, Carolina Family Health Centers, Inc. must comply with the Federal COVID-19 vaccination mandates. Therefore, as of January 27, 2022, new and existing employees to have received, at a minimum, the first dose of the primary series (Moderna or Pfizer) or a single dose COVID-19 vaccine (Janssen), have requested a lawful exemption, or been granted a lawful exemption.

Job Type: Full-time

Pay: $17.75 - $30.00 per hour
