Job description
An award-winning health system in coastal Virginia is looking for an experienced Manager of Inpatient Case Management.
This Magnet designated facility is a regional Trauma Center in Virginia and is part of a system with plenty of room for growth and expansion. This hospital also is a certified stroke center and a leading hospital in the region for women's services, cardiovascular care, neuroscience, orthopedics, and more.
This hospital is looking for an experienced Case Management leader who will report to the Director of Case Management and oversee a team of Registered Nurse Case Managers. This position will be responsible for management of the inpatient care transitions program. The Manager will participate in system development and and implementation of integration of care transitions. The Manager will serve as a mentor and provide professional practice consistent with organizational goals, service, effectiveness, and standards of care.
REQUIREMENTS:
- BSN Required
- MSN (Preferred)
- Specialty certification required within one year of eligibility (ACM, CCM, CCCTM, CMAC
or CMGT-BC). - Active Nursing License
