Job description

MultiCulture Home Care, a Modivcare Personal Care Service, is looking for a bilingual Spanish or Creole-speaking RN Staff Nurse to provide quality nursing care and related services in patients' homes. You will report to the local Branch Manager.

You will... 

• Provides quality nursing care and related services in the home.
• Provides those services requiring skills using the Nursing Process (Assess, Plan, Implement, and Evaluate.)
• Continually evaluates the client's nursing needs and works with the client/caregiver in developing ways to meet their needs.
• Follows a current, individualized treatment plan by making revisions based on original and ongoing assessments and in conjunction with the doctor, case manager, and client/caregiver.
• Informs the physician, case manager, and other healthcare team members of changes in the client'/s condition and needs.
• Initiates and performs teaching of the client, family, and other health team members, as appropriate, to meet identified needs and achieve goals.

We are excited to speak with someone with the following…

• Graduate of an approved school of practical nursing.
• 2 years experience as an RN.
• Prior Home Care experience preferred.
• Ability to provide care in a variety of settings.
• Bilingual in English & Spanish or Creole required

Pay: $40.00-$45.00/ Hourly
