QUALIFICATIONS:
1 Ability to read, write and comprehend English and to follow oral and written instructions.
2. Interest in the needs of the aged and the chronically ill with the ability to work with the elderly.
3. Ability to work hours as scheduled based on the requirements of the position/assignments.
4. Registered Nurse licensed in the State of New York. Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred.
5. Knowledge of nursing theory and practice including the administration of medications.
6. CPR certified.
WORKING CONDITIONS: Works in an appropriately lighted and ventilated environment. This position may result in your being exposed to blood/body fluids.
PHYSICAL REQUIREMENTS:
1. Must be able to move intermittently throughout the day, proper body mechanics required—bending, stooping, turning, stretching, reaching above the shoulders and occasional lifting are involved.
2. Must be able to cope with the mental, emotional and physical stress of the position.
3. Must be able to observe, listen and communicate effectively.
4. Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, support agencies, and outside agencies.
5. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
PRIMARY FUNCTIONS:
1. Responsible for quarterly assessments, the MDS/Rap modules, and input into the care plan process.
2. Oversees all admissions to the facility for JCAHO standards. Ensures all paper work is properly executed (i.e., Admission’s Comprehensive Assessment, MD Orders, lab work, etc.).
3. Ensures weekly wound documentation is appropriately noted.
4. Evaluates, recommends and assists with resident treatments.
5. Assumes responsibilities for providing all CNA’s with their daily assignments.
6. Responsible for maintaining all resident profiles.
7. Makes unit rounds at 2:30 p.m. and 3:15 p.m. with the CNA staff and communicates with unit staff concerning new admissions and acutely/seriously ill residents.
8. Participates in Infection Control Rounds and Safety Rounds of the unit.
9. Maintains the following audits: Medication/Treatment Kardex on daily/weekly/monthly basis; MD Orders, H & P’s, Lab work, PPD’s, AIMS testing signed by the M.D., monthly weights, monthly blood pressures, ADL Flow Sheets BP/FS, Monthly Summaries, Completeness of Restraint Reduction Forms, DNR’s, Antipsychotic Behavior Flow Sheets, Custom W/C Monthly Nursing Notes, and Positioning Schedules.
10. Monitors the Medication Carts for cleanliness, organization, drug/stock availability, discontinued medications, unlabeled and/or expired medications and medication containers.
11. Responsible for nursing care plans from day one admission to day twenty-one.
12. Will assume charge nurse position if there are call-outs leading to short staffing.
13. Checking quarterly assessments for accuracy and completeness.
14. Attends care plan meetings as needed.
15. Responsible for attending all In-services and Educational Seminars as required
16. Assists with planning and the Educational needs of specialty units.
17. Assumes other duties as assigned by the Unit Manager, ADNS, or DNS.
