A Registered Nurse (RN) is a professional member of the personal care team who provides personal care services to clients under an established Physician Plan of Treatment, in compliance with the Nurse Practice Act, and adheres to the policies and procedures of Smart Choice Personal Care, Inc. This position reports to the Director of Nursing.

Smart Choice Personal Care Inc. is a Personal Care Agency that serves clients in their homes. We are looking for Registered Nurses to provide supervisory and training visits in client homes for the Personal Care Worker Program.

Location: Dane County, WI

Rate of Pay: $40.00 total for each supervisory visit, OR $55.00 total for each supervisory visit that includes a Personal Care Screening Tool (PCST) assessment.

*Mileage reimbursement in addition to visit rate.

Visit Information: Approximately 3-15 visits per month. Based on RN preference and availability. Visits may be scheduled flexibly within the parameters of program rules.

Qualifications:

1. Must possess a valid current registered nursing license (RN) under the State Board of Nursing.

2. Must have at least 1 year nursing experience of direct patient care and/or experience with the personal care program.

3. Must be able to manage schedule independently and meet required deadlines.

4. Must possess accurate assessment skills and effective communication skills.

5. Must possess a current driver’s license and have reliable transportation in order to travel between patient homes.

6. Must complete Smart Choice Personal Care, Inc. employment process.

Duties:

1. Assess client condition, safety and Personal Care Worker (PCW) competency.

2. Communicate information and provide training as needed in client homes.

3. Collaborate with other members of the Smart Choice Personal Care Team as appropriate.

4. Complete accurate documentation of training and assessments.

5. Schedule assigned appointments and work with clients, families and PCWs to meet the requirements of the PCW program.

*Paid training will be provided upon hire.

Pay: $40.00 - $55.00 per hour
