CC Young, a Continuing Care Retirement Community and a not-for-profit leader in Senior Housing and Health Care, has an immediate opening for a full-time Scheduler/Certified Nurse Aide to assist our Home Health Department in scheduling patient services, coordinating care with interdisciplinary teams and providing Certified Nurse Aide services for our on-campus residents and off-campus patients.

Job duties for this position include, but are not limited to:

  • Schedules and appropriately document patient schedules.
  • Participates in coordinating care with management and the patient’s interdisciplinary team.
  • Provides information and responds to requests for home services information from families, referral sources and prospective clients.
  • Assists with coordination of logistical support (durable medical equipment, purchased services) and scheduling of care for new and existing patients.
  • Participates in the coordination of interviews and application review for prospective employees.
  • Obtains ongoing insurance authorization for care.
  • Orders medical supplies and maintains inventory for patient’s needs.
  • Assists with Medical Records and ADRs.
  • Performs other duties as assigned.

Minimum qualifications include:

  • Minimum of one (1) year experience as a Certified Nurse Aide (CNA), preferably within the Home Health environment.
  • Strong verbal, written, interpersonal and customer service skills for interaction with all levels of Company personnel, Residents/Patients, Family Members and Agency personnel.
  • Demonstrated computer skills, including Microsoft Office Suite.
  • Ability to conduct off site patient visits.
  • Possession of a valid driver license in good standing in Texas, reliable vehicle for off-campus travel to patient residences and proof of auto liability insurance

Salary: $14.50 - $18.50 per hour
