Job Title: Case Coordinator
Department: Administrative
Reports: Quality Assurance Coordinator (directly) & Assistant Regional Manager (indirectly)
Status: Non-exempt (eligible for overtime pay), full time
Location: Clinic
POSITION DESCRIPTION:
The Case Coordinator will be responsible for scheduling the direct staff to provide services that fulfill the clinical recommendation of the client. In addition, the Case Coordinator will work directly with the client’s families to coordinate an appropriate schedule. The Case Coordinator will inform the client’s families of CARE holidays and collect direct staff and family availabilities. This position is accountable for the on-boarding training for new hires and case assignments for clinical supervisors.
KEY RESPONSIBILITIES:
Essential Duties and Responsibilities include, but are not limited to the following:
- Enter sessions into the electronic software system based on the direct staff’s availability and the client’s availability
- Request personnel as needed
- Attend the weekly Scheduling Meeting
- Based on availabilities, accommodate the client’s schedule, and notify the clinical team of any changes
- Accept phone calls and/or emails from client’s families and make appropriate changes to the clients’ schedule while maintaining close contact with the clinical team
- Coordinate and manage new hires shadowing schedule with therapist, supervisors and clients
- Partner with scheduling team to project client changes due to decrease in hours or terminations
- Calls in advance to confirm scheduled appointments with clients
- Schedule and coordinate for clinical staff
- Lead Scheduling Introduction Training for New Academy Training (NAT)
- Maintain and monitor with staff’s schedule to maximize availability
- Maintains exceptional customer services with clients and with staff
- Prepare and coordinate caseloads for clinic team in partnership and direction with the Assistant Regional Manager, Referrals, and Quality Assurance Coordinator
- Complete the Master Log, cancelation log daily
- Verifies non-billable appointments after completion of each appointment
- Respond to CARE emails within 24 hours.
- Ensure schedule is updated on Rethink/practice management software to reflect any cancellations, make-ups, breaks, meal, and travel time daily
- Perform additional duties as assigned
REQUIRED COMPETENCIES:
- Competency and knowledge in administrative tasks
- Ability to deal professionally with all types of people and maintain a pleasant disposition
- Strong written, oral and interpersonal communication skills
- Must have good organizational skills, and have ability to prioritize, handle and complete multiple tasks within specified time frames
- Must be able to work in a diverse, multi-cultural environment and be sensitive to the service population’s cultural and socioeconomic characteristics
- Must have ability to maintain confidentiality and exercises discretion and good judgment
- Bilingual Spanish a must
MINIMUM QUALIFICATIONS:
- High school diploma or equivalent required
- Minimum 2-years’ experience working in the customer service field
- Minimum 1-year as scheduler and working experience within the field of autism and developmental disabilities
- Minimum 1-year experience in medical or behavioral health
- Must be computer literate with knowledge of Outlook and Microsoft Office software (i.e., Word, Excel) and the ability to learn other programs
- Must be able to work a flexible schedule between the hours of 8-6 pm, Monday to Friday, full-time, 40 hours per week
PHYSICAL/MENTAL DEMANDS: PHYSICAL/MENTAL DEMANDS:
Combination of sitting, standing, bending, light lifting and walking.
- Requires a full range of body motion including manual and finger dexterity and hand-eye coordination
- Requires corrected vision and hearing to a normal range
- Requires the ability to manage stressful situations
- Occasional stress from varying demands

- Visual Acuity. The employee is required to have close visual acuity to perform an activity such as: preparing and analyzing data; viewing a computer screen, extensive reading
- While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms. The employee may be required to periodically stand, walk, sit, stoop, kneel, or crouch. The employee may be required to occasionally climb or balance
- Must have the ability to engage in the physical aspects of approved restraint training
- The employee must occasionally lift and/or move up to 20 pounds, and infrequently up to 40 pounds
- The noise level in the work environment is usually moderate
