Pay Rate Range: $21-22/hour

Job Responsibilities:
The Service Coordinator is responsible for working with clients to assess their needs, coordinate service plans, and enrollment in programs that will better assist them with day-to-day activities and needs. This person will coordinate care and maintain a caseload from start to finish.
Requirements:
- Bachelor's Degree
- Experience with Intellectual Developmental Disabilities community
- Mental health background a plus
