Join our Long-Term Service and Supports Team!
Long-Term Service and Supports Service Coordination will assist individuals living in the community to gain access to services and support that meet their needs, most often referred to as waivers. The LTSS Service Coordinator works together with individuals and their person-centered planning team to identify, coordinate, and facilitate all necessary services regardless of the funding source.

Summary of Responsibilities:
Under the general direction of the Office of Long-Term Living (OLTL) Unit Supervisor, this person provides support coordination services for Participants. Services are provided in accordance with all existing policies, procedures, and regulations and in accord with the mission and values of the company.
Essential Functions:
Complete Authorizations for Services, Referrals
Verify Eligibility
Crisis Intervention
Inter-agency Planning
Provide Support
Attend Court Hearings
Complete Detailed Service Notes
Waiver Monitoring
Complete Person-Centered Service Plans
Complete Freedom of Choice Process
Complete Requests for Services
Remediation Services Resulting from QMET, MCO, and NCQA
Communicate OLTL Priorities to Families
Personal Characteristics:

Education and Experience:
Bachelor's Degree in Psychology, Social Work, or Registered Nurse in the Commonwealth of PA, 3 years' experience in a social service or healthcare setting.
Service Coordinators working with individuals with traumatic brain injury must also have at least one year of experience in rehabilitation and/or working with individuals with traumatic brain injury.
Employment Requirements:
Automobile and Valid Insurance, Valid Pennsylvania Driver's License, acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance
Working Conditions:
Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, daily travel in local county area(s) to various provider and consumer locations, some work may run into the evening hours and during adverse weather conditions, must be able to lift to 15 pounds.
