Job description

The primary purpose of the social worker is to plan, organize, develop and direct overall operations of the Social Services Department. The Social worker will ensure accordance with current federal, state and local standards, guidelines and regulations. They will establish policies and procedures as directed by the administrator to assure that the medically related emotional and social needs of the resident are being met and maintained on an individual basis. The social worker is responsible for providing emotional support, counseling and guidance to residents and their families while helping them navigate the known and unknown of the healthcare system. The Social Worker will work directly with the patient and their family to help educate residents about resources that are available to them, how to obtain those resources, and help them understand their unique needs. A successful Social Worker will have good working relationships with outside providers and be able to communicate effectively and timely with residents and healthcare personnel to get them the needs and services that will be critical to the success of every resident.

Roles and Responsibilities:

· Plan, develop, organize, implement, evaluate and direct the social services program at the facility

· Assess each resident’s unique condition, combining data from medical diagnoses and personal interviews and assessments

· Develop personalized, comprehensive care plans to help residents adjust to medical diagnoses and transition from the health care system to normal life

· Participate in discharge planning, arranging care conferences and transportation

· Provide necessary support for each resident according to health status, personal strengths and weaknesses, emotional state and additional factors

· Research local support groups and other resources to determine which ones are right for individuals

· Evaluate services and resources provided to residents to determine effectiveness

· Follow up with residents throughout their stay to address current state of mental and emotional health

· Maintain current knowledge of local resources and relevant medical research to provide best possible recommendation to residents

Qualifications, Skills and Education Requirements:

· Proven work experience as a Social Worker or similar role

· Excellent written and verbal communication skills

· Ability to explain medical concepts to patients in understandable language

· Compassionate demeanor with patients of all ages and conditions

· Strong organizational, multi-tasking and time-management skills

· Good working relationships with physicians, other healthcare personnel and local home care agencies

· Relevant training and/or certifications as a Social Worker

· Social Worker license in the state of Minnesota (Preferred)

Additional Notes

Minimum of a bachelor’s degree in Social Work is Preferred.

Job Type: Full-time

Pay: $40,000.00 - $45,000.00 per year
