Job Summary
Under the general supervision of the Clinical Supervisor of the Citywide Richardson Team/Rene Casanave Apartments (RCA), the Social Work Associate (SWA) works on-site in the Permanent Supportive Housing building, Richardson Apartments, with formerly homeless individuals with mentally illness, chronic physical health issues, and/or substance abusing clients to prevent homelessness and link to appropriate outpatient services.
The SWA will carry a caseload of residents from Richardson/RCA Apartments and provide individual treatment, including case management services, psychotherapy, crisis intervention and 5150 assessments, substance abuse treatment, and psycho education, group treatment, community events; intake, assessment, service planning and consultation. The SWA will coordinate closely with property management staff from Community Housing Partnership and Richardson/RCA Apartments Citywide Team members to develop successful treatment strategies; advocacy and coordination with other agencies, families and clients; track client contacts in Avatar Billing System, assessments, referrals and linkage; coordinate closely with hospitals and clinic medical providers and other community partners.
Working with diverse and multicultural team and community, the individual will participate and contribute to culturally focused clinical interventions and will practice a commitment to cultural humility and anti-racism. The candidate demonstrates a commitment to anti-racism, social justice and the ability to engage and respect a diverse community of clients and peers. The SWA tracks client contacts, treatment plans, assessments, evictions and makes appropriate referrals. The SWA maintains client records according to California, City and County requirements; and participate in an interdisciplinary treatment team. The candidate attends weekly staff meetings and participate in all required training sessions.
The job will involve the ability to lift 50 lbs., walking and climb stairs on a daily basis.
Work location: One position is located at 25 Essex and the other position is at 365 Fulton.
Department Description
The Division of Citywide Case Management Programs at UCSF/Zuckerberg San Francisco General (ZSFG), Department of Psychiatry is located at 982 Mission Street, San Francisco. The Division provides comprehensive outpatient treatment to those suffering from severe mental illness, often complicated by homelessness, chronic traumatization, institutionalization, substance use disorder and chronic medical conditions.
The mission of the Division is to help clients recover prior functioning, while reducing reliance on acute services, and facilitate transitions from ZSFG psychiatric inpatient, jails, or other institutions to the community. Our programs serve a diverse group of individuals, in terms of social, cultural, and linguistic characteristics; and is structured around interdisciplinary teams. Interventions provided include psychotherapy, crisis intervention, psychotropic medication management, case management, outreach, and employment services.
Required Qualifications
- Possession of a Bachelor's degree in Social Work from a program approved by the Council on Social Work Education, and 1 year of experience; or an equivalent combination of education and experience if a Bachelor’s degree is not possessed; and knowledge and abilities essential to the successful performance of the duties assigned to the position
- 1 year working with severely mentally ill adults, the homeless and dual diagnosed populations
- 1 year of Case Management experience
- The ability to do outreach, including to SRO hotels, inpatient hospitals and scattered site housing locations
- Understanding of mental health issues and sensitivity toward mental health clients
- Ability to work with clients and program staff from diverse cultural and social class backgrounds
- Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology
- Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately, and be detailed oriented

Preferred Qualifications
- Spanish language fluency adequate to provide therapy and clinical services to clients in this language
- Demonstrated ability to work within multidisciplinary team to support clients
- Experience with Avatar Billing System
- Prior related experience in the homelessness response system of SF
- Demonstrated understanding of Harm Reduction, The Recovery Model and Housing First Model

Pay: $27.34 - $60.13 per hour
