All Metro Healthcare, a Modivcare Personal Care Service, is looking for a (2) Social Worker/Case Worker responsible for developing participants' care plan of services to ensure health and safety in their homes in the community. You will report to the Area Director or local Branch Manager.

You will...

  • Responsible for a caseload of 18 clients
  • Required to document when they see participants monthly to ensure services are provided, and they are happy with their service providers.
  • They are responsible for scheduling and facilitating team meetings with all service providers present to review goals and ensure that such goals meet the community participants' needs.
  • Responsible for being knowledgeable of community resources, assisting with medical management, setting up transportation to important appointments, etc.
  • Service coordinators are responsible for maintaining participant records and writing a comprehensive care plan once yearly and/or addendum should their needs change in the middle of the approval period.
  • Service coordinators also help participants locate housing if necessary and address housing-related issues, ensuring ongoing benefits and entitlements are maintained, including but not limited to Social Security, Medicaid, Medicare, food stamps, etc., and request and attend fair hearings to maintain such benefits if necessary.
  • Service coordinators are also responsible for reporting incidents that occur timely to the Department of Health, our auspice, and assisting and cooperating in the event of auditing.
We are excited to speak to someone with the following…

  • Master's degree and a 1 year of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors; or
  • Bachelor's degree and 3 years of experience providing case management/service coordination, information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.

Schedule: M-F 8:00AM-5:00PM

Office: Remote in New York, but able to see 18 clients every month in the 5 boroughs
