We are currently looking for a Substance Use Specialist for our ACT team.

General Description

Provide motivational interviewing, education and counseling on mental health and substance abuse issues to clients in individual or group settings.

Job responsibilities:

• Act as a member of the treatment team which shares caseload responsibilities; within the team, however, the Substance Use Specialist will be the primary program manager of a portion of the caseload
• Provide community support to people with serious mental illness or serious mental illness and substance abuse issues at varying levels of intensity based on individual client needs for assistance in meeting individual goals including monitoring medical and psychiatric treatment,

assuring coordination and continuity of care during and after re-hospitalizations, linking the client with supportive community services enabling the client to become as self-sufficient as possible

• Provide 60% of each work day to billable client services
• Maintain high quality achievement of targeted results through the use of performance management and the principles and practice of continuous quality improvement

• Write and implement Individualized Treatment Plans with client; provide ongoing assessment of client’s functioning level and, when necessary, make appropriate contacts for change in level or type of service needed
• Assist the treatment team with assessment and treatment planning of clients with co-occurring issues outlining the stage-wise interventions consistent with stage of treatment
• Utilize the treatment team experience to assist clients with co-occurring issues especially non-responders who require secondary interventions
• Provide cross training about substance abuse issues to other team members
• Duties shall also include routine and regular visits with clients in and out of their apartments or other community locations, crisis intervention and resolution, problem solving, and assistance with activities of daily living skills (shopping, managing money, using public transportation, etc.) to facilitate personal growth and independence
• SAS will assure that assigned case records report service activities accurately, comprehensively, and in a timely fashion
• Provide on-call crisis and back up interventions and services
• Complete yearly training requirements as outlined in PfP policies
• Perform other related tasks/jobs as assigned by the supervisor

Job Qualifications

Requirements:

For Non ACT Teams:

Bachelor’s Degree in a social services field and Substance Abuse Certification. Experience may be used in lieu of degree with approval of HR and accrediting bodies as appropriate and necessary for agency compliance. Med Tech Training, First Aid, CPR, and valid Chauffeur’s License

For ACT Teams:

Bachelor’s Degree with licensure or an advanced Substance Abuse Certification. Med Tech Training, First Aid, CPR, and valid Chauffeur’s License.

Physical Requirements: Ability to stand, bend, kneel, lift (20-30 lbs), navigate steps or use appropriate accommodations, drive and be able to sit for periods for hours at a time.
