Position Summary:

This position is responsible for coordinating daily activities for residents living in a Supportive Housing site. This position will provide 20 hours of on-site coverage at the site to maintain a visible presence at the site and provide information and support to residents. This position will also provide case management to residents at the site and complete a small portion of community intakes.

Principal Accountabilities:

Coordinates the Supportive Housing Program

  • Provide 20 hours off on-site coverage at the site.
  • Coordinate weekly activities for residents.
  • Collaborates with the local Housing Authority and assigned vendor in the building.
  • Conducts assessments and coordinates comprehensive services for residents.
  • Partners with vendors to implement services and provide 24-hour access to service for residents.
  • Maintains a regular, visible presence at the supportive housing site.
  • Provides information and support to housing residents.
  • Plans and arranges activities for residents within the framework of the program budget.
  • Completes calendars and flyers for events and activities monthly to distribute.

Completes intakes and manages cases of consumers living at the Supportive Housing site.

  • Conducts a comprehensive evaluation to determine the full scope of clients’ needs.
  • Determines eligibility for programs.
  • Develops appropriate service plans that meet consumer choice and budget guidelines.
  • Partners with vendors to implement home care services.
  • Monitors clients for changes requiring adjustments to service levels.
  • Documents phone calls, visits, and all activity in accordance with Elder Affairs’ and agency requirements; maintains client records accurately and within the mandated timeframe.
  • Conduct intakes for consumers in the community as needed.

Qualifications:

Education: Bachelor’s degree in human services or related discipline preferred. Applicants with an associate degree may be considered if they have experience in activity planning or related skills.

Experience: Case management background helpful. Experience with organizing group activities beneficial.

Special Skills/Knowledge: Excellent oral and written communications skills, ability to effectively relate to a wide variety of personalities. Ability to engage individuals in participation of group activities. Basic computer skills, organizational and time management skills as well as creative skills.

  • Driver’s license and reliable transportation required.
