Position: Group Facilitator
The Group Facilitator will provide direct patient care to patients with substance use and behavioral health disorders. The Group Facilitator will ensure the safety and well-being of these patients by maintaining a safe milieu. The Group Facilitator will also participate in treatment planning and implementation. They will also facilitate groups within assigned treatment programs, and document according to department/organization procedures.
Job Duties:
- The Group Facilitator will Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served.
- This includes knowledge of substance use disorders and how it impacts the physical and psychological needs of the patients as well as the ability to respond appropriately to those needs.
- Knowledge of the different stages of recovery and how this contributes to a patient’s individualized need.
- This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.
- The Group Facilitator will participate in treatment planning by working with the interdisciplinary team to identify the patient's problems, needs, and strengths,
- The Group Facilitator will suggest modifications to the treatment plan based on observation of patient behavior and provides input into discharge planning and identify the impact of Substance Use on patient needs.
- Monitors and reports patients’ progress and communicates information to charge person on assigned shift and family members.
- The Group Facilitator will facilitate didactic, educational, and unit/program community groups within the assigned treatment programs.
- Provides Chemical Dependency Education for patients in a Chemical Dependency or Dual Diagnosis Program.
- Encourages and facilitates patients’ participation in therapeutic activities. Supervises patients to ensure their safety and their involvement in treatment.
- The Group Facilitator may transport (in a company-owned vehicle) and/or accompany patients as assigned by the charge nurse.
- Unit rounds are conducted, and precautions records are completed in real-time. Precautions records are accurately labeled.
- The Group Facilitator will complete patient, program, and department documentation.
- The Group Facilitator will orient new patients to the unit, program, and milieu by establishing a rapport with patients and families and making necessary introductions, and providing appropriate hand-off information to the treatment team regarding the patient’s needs.
- Ensures patient safety by conducting accurate safety rounds per organizational policy. Conducts safety check and contraband search on admission, as needed per hospital policy.
- Maintains a safe, orderly, and therapeutic physical environment for patients.
- Performs clerical and administrative assignments to support patient care, department, and organization operations.
- Attends mandatory orientation and training activities within the program.
- Complies with facility safety, infection control, and security program.
- Assists with specimen collection if needed.
Qualifications:
- Preferred Masters Degree Student or pre-licensed MSW
- Bachelor’s Degree is in a Behavioral Health-related field with CADC certification
- Bachelor’s Degree is in a non-Behavioral Health-related field, with a minimum of one year of behavioral health experience and CADC certification
- Experience working with substance use disorder population
- Maintain information as highly confidential
- Familiarity with medical and psychological terminology
- Skills in teaching or assisting with activities of daily living
- Prior experience with therapeutic limit-setting and milieu management
Requirements:
- A valid driver’s license
- Current CPR Certification issued by either American Heart Association or Red Cross
- Current CADC or has an interest in pursuing CADC
- Knowledge of Substance Use Disorders and Co-Occurring Mental Health Disorders
- Knowledge of Dialectical Behavior Therapy, Acceptance and commitment therapy, Motivational Interviewing, and Cognitive Behavioral Therapy
