Description:

The Care Manager (CM), as a behavioral health division team member, will work in collaboration with Homeless Program Provider and support staff in providing client-centered services that implement the clinical treatment plans, including providing direct patient care within the scope of training/licensure, legal/ethical/regulatory standards, and organization policies and procedures. The CM will assist in linking clients to programs for support services as needed. These services ensure timely and coordinated access to appropriate levels of health service and continuity of care through ongoing assessment of the client. This is a professional position working in the development and coordination of public health education, health promotion and community outreach activities. The CM must be able to work a flexible schedule based on workload and events which may include weekends and/or evening work. The CM will serve as a primary resource for ECCHC’s Health Care for the Homeless (HCH) Program.

Principal Responsibilities:

  • ~50% behavioral health consulting
  • ~40% care management
  • ~10% data management
  • Provides brief behavioral health consultations (initial and follow-up) to clients and curbside consults to primary care providers.
  • Promotes the coordination, collaboration, and continuity of care for all clients requiring care management or therapy. In addition, the care manager will help clients navigate different insurance and public assistance programs available to them.
  • Works closely with partnering organizations to help coordinate the medical care of clients. An understanding of the homeless population, barriers to care, disease prevention and transmission, and sensitivity to the social stigma that exists for homeless patients is critical.
  • Interacts closely with the Program Coordinator to determine funding availability, care interventions and utilization of resources within the client’s care network. Provides routine research related to public and private funding opportunities for community outreach projects, and assists in writing proposals for additional program funds. Travels to regional and national meetings to enhance professional development skills and network with colleagues and potential funders.
  • Assist patients with multiple co-morbidities, complexities, and diverse needs.
  • Interview clients, coordinate, and plan linkages to programs and activities to meet their social and/or emotional needs.
  • Provide referrals to appropriate community resources as appropriate; follow up as needed.
  • Develop and update written protocols for the delivery of psychosocial support services.
  • Provide consultation, technical assistance and/or delivery of direct client psychosocial support services as needed, including individual/group therapy.
  • Educate team members regarding the client's attitudes, behaviors and coping styles which may affect their approach to care.
  • Act as the client's advocate in procuring local, state and federal benefits and services which they may be able to receive.
  • Take responsibility for being knowledgeable about and aligned in a working relationship with auxiliary service providers.
  • Work closely with partnering organizations to help coordinate the care of clients experiencing homelessness or transitional living.
  • Develop and maintain a resource and referral file to assist patients in finding timely and appropriate community-based programs and resources.
  • Abide by HIPAA as well as patient, organizational, and practice standards for confidentiality
  • Attend staff meetings, other work-related meetings, training, and perform other related job duties as required or requested.

Education & Experience:

  • Fully licensed in Social Work, Psychology, Counseling, or other related field required
  • Understanding of diagnosis using DSM 5 and relevant evidence-based treatment options
  • Experience working with grant programs preferred
  • Excellent communication skills both written and verbal.
  • Understanding of the homeless epidemic, barriers to care, disease prevention, and sensitivity to the social stigma surrounding the homeless population.
  • Three to five years care management with the homeless population preferred.
  • Understanding of the health care system and its various components.
  • Basic understanding of Medicaid/Medicare.
