The REACH Program of Evergreen Treatment Services works with individuals experiencing homelessness and behavioral health conditions to help them achieve stability and improved quality of life. Through outreach, relationships, advocacy and bridging gaps, REACH focuses on reducing harem and supporting healing. REACH provides outreach-based-care coordination, multidisciplinary clinical services, and supports to access and maintain housing. All services are based in principles of harm reduction that offer respect and dignity to individuals moving forward through stages of change in their lives.

Job Summary
The function of this position is to provide outreach, engagement and intensive case management/care coordination to Spanish-speaking individuals who are experiencing substance use disorders, homelessness, and face barriers to accessing needed resources. The pupose of these services is to help individuals achieve psychosocial stability and improved quality of life. Client-centered services are provided from a harm reduction perspective utilizing motivational interviewing skills. The case manager must develop collaborative relationships with staff of other agencies providing services to this population in order to effectively coordinate care. The nature of this position is extremely independent requiring self-motivation and time management skills, creative thinking, multitasking and conflict resolution skills.
All new hire employees, starting after August 16th, 2021 are required to receive a COVID-19 vaccination as determined by the CDC. New hire employees not in compliance with this policy will not be able to start until vaccination is determined by the human resources department. Additionally, Evergreen Treatment Services will be following the state mandate for all staff to be vaccinated for COVID-19.

Job Functions (May Include)
- Provide outreach, engagement, intensive case management and care coordination services in English and Spanish for assigned clients in a culture-informed manner with particular sensitivity to issues specific to immigrants and refugees:
- Meet clients where they are, either on the streets or at other service facilities (including jail), to explain offered services and establish a working relationship.
- Develop rapport and a trusting relationship with clients through respectful outreach and client-centered engagement practices.
- Develop and implement in collaboration with the client, an Individualized Service Plan which addresses the client’s stated needs and priorities. Review and update this plan regularly to reflect progress or attainment of articulated goals and the emergence of new client needs and to help the client move toward the achievement of autonomy.
- Assist clients in obtaining housing and maintaining housing stability.
- Assist clients with establishing and maintaining primary medical care and addressing chronic health conditions.
- Assess nature and severity of clients’ substance use issues and match clinical interventions to each individual’s stage of change, focusing on reducing the harmful consequences of substance use through self-change methods.
- Address clients’ needs for mental health and substance use disorder treatment services and facilitate engagement in appropriate behavioral health supportive services.
- Assist clients in achieving financial stability through access to entitlement programs, protective payeeships, educational and vocational programs, and employment opportunities.
- Develop and maintain productive working relationships with community programs which may support clients to provide effective coordination of care.
- Provide services consistent with ETS/REACH program policies and procedures.
- Assist clients in keeping appointments.
- Assist clients in developing a spending plan and in shopping.
- Advocate for the client with a wide variety of other service providers:
- Assist with accessing needed services by setting up and attending appointments with clients in order to provide continuity of care.
- Identify gaps and barriers in available community resources and advocate for systemic changes.
- Attend REACH team meetings and other required meetings.
- Utilize clinical supervision, behavioral health consultation and peer support for exploring clinical intervention strategies, counter-transference, resource development, self-care and burnout prevention.
- Develop and maintain client files for assigned caseload according to Agency, contract and state requirements.
- Local travel on behalf of the Agency is a job requirement.

Requirements:Qualification Requirements

Education*
- Bilingual in Spanish and English; at a minimum, the ability to understand and to make one’s self understood to all Spanish speaking individuals
- High school diploma or equivalent required, undergraduate degree preferred.
- Academic training in the social service field and in the area of substance use disorder treatment desirable.
Experience
- A minimum of two years’ experience in work related to social work or outreach programs.
- Experience providing services to individuals with substance use disorders from a harm reduction perspective is essential.
- Experience with case management, homelessness and working with individuals with co-occurring behavioral health conditions preferred.
- Experience working with non-English speaking immigrants and/or refugees desired.

- Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change.
- Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner.
- Ability to develop and maintain basic budgeting and accounting systems that function in a transparent manner.
- Dependable, able to work under pressure, receptive to change, willingness to learn, cooperative approach to problem solving.
- Good basic clerical skills, including typing, ability to proof own work, good grammar and spelling.
- Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic and cultural backgrounds; good diplomatic skills.
- Ability to work very independently and seek supervision when appropriate.
- Possession of a valid Washington State Motor Vehicle Operator’s License (local travel on behalf of the Agency is a job requirement) and good driving record preferred.

Language Skills
- Ability to read and interpret general business correspondence, policies and procedures, referral information, financial documentation and applicable government regulations.
- Ability to write business letters, uncomplicated reports, instructions and procedures.
- Ability to present information effectively and respond to questions from patients, staff, referral sources and the general public.
- Ability to do all the above in English and Spanish.
Mathematical Skills and Reasoning Ability
- Thorough knowledge of and ability to apply business arithmetic skills accurately and rapidly.
- Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may be limited.
- Ability to interpret a variety of instructions furnished in written, oral, schedule or diagram format.
