REPORTS TO: Case Management Supervisor
SALARY: Full benefits, PERS of WA Pension Plan
WORK HOURS: 40 hours per week, Non-Exempt
OFFICE LOCATION: 1 position based in Aberdeen
Duties
RESPONSIBILITIES: Provides in-home Case Management services to long-term care residents within Grays Harbor County. Provides assessments, creates service plans. and coordinates with partnering service providers, to provide the best care focusing on independence and individualization. Qualifications
Requires a Master’s degree in behavioral or health sciences and one year paid on the job social services experience,
OR Bachelor’s degree in behavioral health sciences and 2 years paid on the job social services experience,
OR BA and 4 years paid on the job social services experience. Well-developed human relations skills and ability to work in a team-based environment. Knowledge of social service or human service issues, especially dealing with adults with disabilities.
Must have reliable personal transportation, vehicle insurance, and a WA driver’s license.
