We are seeking competent, compassionate, and career-oriented therapists to serve as a Service Facilitator/Care Coordinator.

Job Purpose:

Our next CCS Service Facilitator/Care Coordinator will responsible for community-based family support for mental health and behavioral needs of youth and young adults. Our next CCS Service Facilitator/Care Coordinator will support families through community resources and a strengths-based approach to help and healing.

Duties:

  • Performs clinical assessments, including intake interviews and screens for new referrals using trauma informed and recovery-oriented perspective.
  • Develops, directs and implements client treatment plans, interventions, skill training and crisis management.
  • Creates appropriate expectations for family involvement upon admission.
  • Encourages family investment and participation in treatment planning and ongoing treatment.
  • Develops a network of community support and resources and connects clients with these resources as appropriate.
  • Demonstrates relationship building and information sharing skills while supporting a collaborative system of care.
  • Establishes and maintains a working knowledge of the WCHSD programs, resources, and supports.
  • Completes all necessary paperwork timely and utilizes trauma informed, strength-based language.

Qualifications:

  • Bachelor's Degree in Social Work or a related field required; Master's degree preferred.
  • Ability to work in-home with children and families, as well as coordinate services with ancillary providers within the community.
  • Ability to multi-task and prioritize work within the community; in conjunction with timely completion of documentation to support the work completed in the community.
  • Ability to take initiative and act quickly and independently when the situation demands it; sees a need and takes appropriate action without being prompted or reminded; demonstrates ability to effectively deal with crisis situations.
  • Ability to exhibit professionalism while working effectively and harmoniously with staff, program participants, social service agencies, legal parties, and the public.
  • Demonstrate an ability to effective understanding and practice of cultural humility and ability work and communicate respectfully with individuals from diverse backgrounds.
  • Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
  • Valid Wisconsin driver’s license required.
  • Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
