Community Care Hospice is hiring a Chaplain.

Community Care Hospice is being formed to serve the Hutchinson, Newton and McPherson area through a partnership between several non-profit retirement communities and Midland Care Connection.

Summary of Position

The purpose of this position is to provide support, advice and counsel to patients and their families on spiritual matters while assuring compliance with agency policies and procedures. The Chaplain starts by meeting the patient and/or family member where they are related to their spiritual and/or religious beliefs. This position is responsible for completing spiritual assessments according to state and federal requirements in a thorough and timely manner, as well as documenting all patient and/or family contact in the Electronic Health Record (EHR) per agency procedures. This position is supervised as designated on the agency organizational chart.

Pay Rate - $22+/hour (pay is based on requirements and like experience)

Part Time within the hours of Mon-Fri, 8am-5pm

Duties & Responsibilities

  • Identifies the need for consultation with other members of the Interdisciplinary Team (IDT) or with experts in specialized areas of spiritual care or psychosocial practice.
  • Identifies when spiritual/religious beliefs may impact the physical and psychosocial care provided by other team members and alerts team members to this assessment.
  • Contacts the patient’s/family’s preferred spiritual/religious advisor to ensure ongoing involvement and to invite participation in the interdisciplinary care planning.
  • Develops and documents the plan for spiritual care, in conjunction with the IDT>
  • Guides the IDT in provision of spiritual care when Chaplain services have been declined.
  • Collaborates with the IDT in evaluation and revision of the plan for spiritual care of the patient/family unit.
  • Identifies need for intervention by other members of the IDT on an ongoing basis; collaborates in the planning of such care.

Qualifications

Required: Master’s in Divinity (M.Div.), two units of Clinical Pastoral Education (CPE) or Ordination by a Faith Community. Must possess a valid Kansa Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard. Availability of reliable transportation. One year of experience providing spiritual/religious support.

Preferred: Previous experience working with issues related to death and dying.
