Job description
- Supervise staff that calls your patients monthly and as needed.
- Establish a relationship and build credibility with the patient and their families.
- Effectively communicate with patient's providers and broader care team.
- Ensure that patient is receiving the appropriate care, guided by provider's care plan and your experience.
- Provide ongoing education to help the patient better manage their chronic illnesses.
- Create and update comprehensive care plans and care summaries.
- Document all communication and coordination of patient care.
- Follow all privacy policies and maintain the confidentiality of protected health information.
