Overview

The Clinical Manager for the Community Health Outreach program is responsible for the overall clinical and non-clinical Community Health Outreach program operations and implementation and reports to the Director of Community Outreach and Engagement. Develops and manages outreach events promoting screening and early chronic disease detection and access to care in the community. This includes financial management, staff development, training and supervision of community health workers and clinical staff, caseload management and monitoring compliance with System Health Solutions, IU Health and State policies and regulations as well as funding policies and regulations. Responsible for development and implementation of clinical protocols and appropriate documentation. Focus will be around addressing social determinants of health. This role will work in medically underserved, diverse communities with a focus on health equity. Provides clinical oversight lending their clinical expertise. The Clinical Manager will liaise with the integrated social work team and other members of the care team to ensure patients’ medical and social needs are met.

This position works Monday-Friday, with weekend expectations 1-2x per month for staffing and/or overseeing community events.

• Requires a Bachelor of Nursing (BSN).
• If bachelor degree not in nursing, must have Master of Nursing (MSN).
• Master’s degree in Nursing or related field preferred.
• Must be passionate about working with medically underserved, diverse communities.
• Specialty certification related to nursing practice or leadership is preferred.
• Required current State of Indiana licensure as a Registered Nurse.
• Required Basic Life Support certification through the AHA.

• Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
