Requirements:
This position will support quality resident care in all Pine Haven Christian Communities’ assisted living programs.
This position will report to the Administrator of the Haven Drive Campus.
Role and Responsibilities:
- Conduct nursing assessments of potential residents and work with the assisted living managers to determine appropriate placement within the continuum of care
- Responsible for implementation of policies and procedures
- Conduct audits of clinical procedures and participate in quality improvement initiatives
- Support assisted living managers and nursing teams regarding clinical care of residents
- Participate in clinical aspect of Individual Service Plan (ISP); attend resident ISP meetings when necessary
- Assist managers in investigating, documenting, and following up on clinical care concerns regarding residents
- Communicate with physicians, hospital staff, clinical and social services staff regarding residents
- Partner with the Nurse Educator/Occupational Health nurse to identify educational needs based on quality audits
- Support required monthly meetings for front line staff through attendance
- Responsible for resident infection control monitoring throughout CBRF and RCAC facilities
- Coordinate and administration of the annual flu vaccine and other vaccines for residents in all assisted living environments
- Serve as a member of the Clinical Quality Team
- Provide after hours on call support to assisted living managers and participate in on-call rotation with managers
- Act as an Assisted Living RN/Nurse Delegator
- All other duties as assigned
Qualifications include:
- Current Wisconsin RN license and CPR certification
- Prior experience in aging services required, preferably in an assisted living environment
- Strong communication skills and teamwork are critical
