Carolina Family Health Centers, Inc. (CFHC) is seeking a proficient, self-motivated individual for this excellent opportunity.

This position works a 40-hour work week, Monday-Friday, in a primary care setting. The Clinical Services Pharmacist must be able to travel regularly among various CFHC sites.

The Clinical Services Pharmacist is responsible for the provision of quality patient care through collaborative drug therapy management and disease state education. The Pharmacist works collaboratively with the health care team and patients to optimize medication therapy as well as quality outcomes.

The Clinical Services Pharmacist communicates drug information, provides comprehensive medication management, assists with medication access, optimizes preventative care, supports seamless transitions of care, and provides education to patients and providers. The Clinical Services Pharmacist is also responsible for the safe, efficacious, and cost-effective dispensing/distribution of medications at all of CFHC’s pharmacies, floating to non-primary sites based on staffing needs. The Clinical Services Pharmacist is responsible for abiding by all statutes, regulations, and CFHC, Inc. approved policies and procedures pertinent to the practice of pharmacy.

The Clinical Services Pharmacist must be a team player with superior written and verbal communication skills and must also be able to work independently on patient care and projects as designated. The Clinical Services Pharmacist must be able to travel regularly among various CFHC sites. Position requires frequent standing, sitting, and comprehensive computer literacy skills with an ability to navigate multiple software programs efficiently and effectively. This position reports to the Chief Pharmacy Officer. This position exercises supervision on assignment, direct supervision to pharmacy technicians; when not assigned, indirect supervision to pharmacy department employees as required.
Minimum Qualifications:

• Graduation from an accredited College or School of Pharmacy required
• Pharmacist currently licensed by and in good standing with the NC Board of Pharmacy required
• Minimum of one (1) year of year of pharmacist experience and experience working collaboratively with an interdisciplinary health care team required
• APhA Pharmacy-Based Immunization Delivery and BLS Certifications required
• Completion of a post-graduate residency and eligibility for Clinical Pharmacist Practitioner (CPP) preferred
• Board Certified Pharmacotherapy Specialist (BCPS) or Board-certified Ambulatory Care Pharmacist (BCACP) a plus
• Bilingual a plus
• Valid NC driver’s license required

Physical Requirements:

• Frequent sitting, occasional bending, stooping, lifting, reaching forward and overhead
• Almost constant standing and walking
• May occasionally be required to lift patients, office equipment and paper, usually not exceeding fifty pounds of direct lift
• Frequent computer work required
• Must be able to travel freely in the local area

As a Medicaid/Medicare facility, Carolina Family Health Centers, Inc. must comply with the Federal COVID-19 vaccination mandates. Therefore, as of January 27, 2022, new and existing employees to have received, at a minimum, the first dose of the primary series (Moderna or Pfizer) or a single dose COVID-19 vaccine (Janssen), have requested a lawful exemption, or been granted a lawful exemption.

Job Type: Full-time

Pay: $108,000.00 - $135,000.00 per year
