Certified /Geriatric Nursing Assistant

JOB SUMMARY:

The Certified Nursing Assistant or Geriatric Nursing Assistant provides personal care and related services in the home. All functions performed are under the direction, instruction, and supervision of the Registered Nurse and Clinical Manager. The CNA or GNA reports directly to the Director of Clinical Services.

RESPONSIBILITIES:

Tasks to be performed by a CNA or GNA must be assigned by and performed under the supervision of a RN who will be responsible for the patient care provided by the CNA or GNA. Under no circumstances may a CNA or GNA be assigned to do any intravenous procedures, procedures involving the use of Nasogastric Tubes, or Foley catheters with the exception of emptying, or any other sterile or invasive procedures, other than rectal temperatures or enemas. They must never administer medications. They are only to do medication reminders.

Except as otherwise noted, duties of the Certified Nursing Assistant or Geriatric Nursing Assistant may include:

  • Helps the patient to maintain good personal hygiene.
  • Assists in maintaining a healthy, safe environment.
  • Plans and prepares nutritious meals.
  • Go to the market.
  • Assists the patient with ambulation.
  • Assists the therapy personnel as needed with rehabilitative processes.
  • Assists with medications ordinarily self-administered (remind only).
  • Assists in household services essential to health care in the home.
  • Taking and charting vital signs and recording intake and output.
  • Encourages the patient to become as independent as possible according to the nursing care plan.
  • Attempts to promote patient’s mental alertness through involvement in activities of interest.
  • Gives simple emotional and psychological support to the patient and other members of the household.
  • Establishes a relationship with patient and family which transmit trust and confidentiality.
  • Prepare a daily report of her visit on the day it is performed and submits same in the clinical record weekly or as directed.
  • Reports any change in the patient’s mental or physical condition or in his home situation to the Clinical Director or the RN.
  • Carries out the assignment as documented in the Care Plan and/or as instructed by the nurse or the therapist and reports to the Clinical Director when she is unable to do so.
  • Works with personnel of other community agencies involved in the patient’s care as directed by the Clinical Director.
  • Performs routine housekeeping tasks that are related to a safe and comfortable environment for the patient as instructed by the DCS.
  • Attends in-service as required by state and federal regulations.
  • Confirming on a weekly basis, the scheduling of shifts/visits with the Client Services Coordinator/Clinical Director, to coordinate necessary visits with other personnel.
