Job Description
The Collaborative Care Model Collaborator (CoCMC) is a core member of the collaborative healthcare team. The CoCMC coordinates the overall effort of the healthcare team and ensures effective communication among team members. The CoCMC is responsible for supporting and coordinating the physical and behavioral health care needs of patients assigned to their caseload with the patients' medical provider, consulting psychiatrist and, when applicable, other healthcare members and/or behavioral health providers. The CoCMC is a part of the healthcare team and establishes and maintains a relationship with community agencies, has a working knowledge of community and financial resources, and assists with appropriate agency referrals and short and long-term planning as needed.
Qualifications
Education:
Medical Assistant from accredited institution with experience in a behavioral health setting. (psychology, social work, helping professions) required. Bachelor's degree from accredited institution with focus on psychology, social work, helping professions or equivalent experience required.
Experience:
3 years minimum of working with individuals with behavioral health (mental health and substance use) needs required. 5 years working with individuals with behavioral health (mental health and substance use) needs preferred.
