A bit more about this role:
Community Guide Case Manager are key members of Devoted’s comprehensive care team. As a Community Guide Case Manager, you will advocate for our members with complex social determinants of health needs. You will be working with members and their caregivers to identify and reduce non-clinical barriers to care they need to sustain a certain quality of life. You will learn a lot about the community environment and how it supports (or does not support) the member’s goals. Based on your local expertise, you will use social services and community programs to support our members in the best possible way.
This is a fast paced company that will come with ebb, flows, and last minute changes. A successful candidate will be outgoing, detail-oriented, and have a natural desire to help people. We often require management of several tasks at once so enthusiasm and organization are key.

Responsibilities will include:
  • Establish trust and build strong relationships with our members
  • Create personalized care plans for members tailored to members’ needs
  • Identify and catalog available community or national resources (i.e., transportation, financial, food, medication discounts, support groups)
  • Connect members to community services and follow up to ensure those services were received
  • Help members (especially low income members) get the resources/care they need by screening for eligible government benefits and programs
  • Work with family members/caregivers to help them in understanding the member’s needs and how they can be supportive
  • Coordinate closely with other Devoted teams like Clinical Guides, Devoted Medical Group, and Member Service Guides
  • Form strong relationships with local community-based organizations and PCP offices to aid with coordination between member, Devoted, and providers (both community and health-related)
  • Identify and catalog available community or national resources (i.e., transportation, food, housing, medical care, or utilities); connect members to community services and follow up to ensure those services were received

Desired skills and experience:
  • You have a minimum of 3 years doing case management or care coordination
  • A Licensed Social Worker or degree in mental health/psychology preferred
  • Embrace, crave, and be comfortable with new and ever-changing technologies
  • Bilingual in English and Spanish or Haitian-Creole preferred
  • Health insurance experience preferred (Medicare Advantage a plus) Experience working with seniors a plus
  • You’ve had proven success in building relationships
  • You’re a great team member with a can-do attitude; you’re self-reflective
  • Ability to work in a start-up, fast-paced environment
  • Embrace, crave and be comfortable with new and ever-changing technologies
