Job Role Summary

This position is responsible for providing overall office support; data entry and management duties and, programmatic assistance and outreach as a member of the chronic disease team that provides services to individuals, families and worksites, coalitions and community agencies. Demonstrates excellent customer service in all interactions. Cross training for other assignments may occur as job related activities dictate. Traveling to sites such as schools, churches, worksites, and other community agencies is required.

Essential Duties

  • Provide outreach services as a member of the chronic disease team.
  • Participate in a variety of community events for the purpose of distributing program information and recruiting coalition members.
  • Assist contacts to access Marion County Public Health Department resources and make appropriate referrals.
  • Distribute fliers/posters/materials within the community.
  • Data entry and management duties.
  • Process client records during health fairs, screening events, and ABCs of Diabetes classes.
  • Enter program data within pre-determined time periods.
  • Continually update knowledge of the Insight Software and emerging data management and communications technologies.
  • Works with MCPHD committees to improve/upgrade data management capabilities including new technologies.
  • Serves as trainer for members of chronic disease team to utilize current and emerging data management systems and technologies.
  • Provide overall office support.
  • Acquires and maintains a thorough knowledge of program and corporate policies and procedures.
  • Maintain an inviting, professional reception area and assist visitors and callers.
  • Manages operational matters such as purchasing, mileage, travel, intra-office communications, and other general office support.
  • Provide specific program assistance and support to maintain and build program capacity.
  • Contact and provide program information to neighborhood agencies, community groups and other pre-determined entities.
  • Create and implement registration mechanisms for events, educational classes, and other program activities.
  • Participate in periodic staff meetings, team meetings, and committee meetings.
  • Maintains confidentiality of information: (a breech is reason for termination)
  • Transports confidential protected health information between worksites, community events, and/or professional meetings.
  • Participates in public health emergency preparedness exercises and in the response to public health emergencies, as directed.
  • Participates in a minimum of two H.O.P.E. events totaling a minimum of eight (8) hours annually.
  • Other duties as assigned.


Other Duties and Responsibilities:

  • Functions as team member along with other Chronic Disease Program staff.
  • Assumes responsibility for professional self-growth and continued development in relevant public health practices.

Qualifications

  • High School Diploma with appropriate math and English coursework; 1 to 3 years of previous work experience, preferred experience in health related field.
  • Current Indiana driver’s license and use of automobile.

Knowledge, Skills & Abilities

  • Ability to concentrate in an open office environment
  • Ability to draw blood and take blood pressure following training
  • Complete paperwork and data entry in a timely and accurate manner
  • A high level of competence in utilizing a variety of computer software
  • Cognitive ability to add, subtract, multiply and divide using whole numbers, fractions, and decimals.
  • Ability to compose simple but professional communications and reports

Working Enviornment

  • Primarily office environment; class instruction and/or outreach activities in varied public sites throughout the county.
  • Physical demands include lifting medium weight materials on a routine basis.
