SUMMARY
The Quality Data Specialist functions as an integral member of the patient care team, providing a high level of customer service to both clinical and administrative staff. This position is responsible for creating and distributing reports for daily operations, quality measures, and high-risk patient identification.
RESPONSIBILITIES:
- Generate and assist with reports, including but not limited to monthly clinical reports.
- Provide support for federal and state level reporting, including Universal Data System (UDS).
- Provide support for accreditation reporting requirements such as Patient Centered Medical Home (PCMH) including aggregating/summarizing the following:
- Patient Satisfaction Survey data, including for vulnerable patients
- Employee Satisfaction Survey data
- No-show and Appointment Availability Tracking
- Patients with Appointments with PCP
- Timely Clinical Advice (i.e. patient cases)
- Printing of Clinical Summaries
- Patient Demographic Data (Age, Gender, Race, Ethnicity, Language, etc.)
- Clinical Data (Allergies, Blood Pressure, Height, Weight, etc.)
- Identifying individual patients for Care Management
- Clinical Quality Measures and CQI including Immunizations,
Preventative Measures, Chronic Condition Measures, etc.
- Resource Usage and Coordination of Care
- Create visual presentations of data by using Pivot Tables and/or Graphics in MS Excel, as needed.
- Provide support for special projects, when requested, such as analyzing data, conducting chart reviews, etc.
- Assist in exporting Excel worksheets and/or capturing screenshots (from Athenahealth), as needed.
- Generate and distribute DARCI (Daily Appointment Report with Clinical Indicators) to clinical staff each day (for next day appointments) for pre-visit planning purposes.
- Assist with the distribution and summarization of Peer Review.
- Performs chart reviews to extract data for necessary reporting.
- Provides outreach to patients, including scheduling appointments, providing education on the importance and benefit of completing quality measures, and following-up on patient satisfaction survey results.
- Provides feedback/input to representative on the Quality Improvement Committee and/or submit Performance Improvement Form(s) to share ideas on improvement of workflow, improvement of patient care experience, etc.
- Maintains confidentiality of all patient information according to federal guidelines and regulations.
- Performs other related duties incidental to the work described herein.
Education, Essential Skills and Experience:
- High school diploma or equivalent; Associate’s Degree or Bachelor’s Degree, preferred
- 3 years of clinical experience, preferred
- Proficient with MS Office Suite – including MS Word, MS Excel (to include pivot tables, tables and graphs), and MS PPT
- Experience in a primary health care setting, preferred
- Strong customer service focus
- Effective verbal and written communication skills
- Teamwork orientation
- Organized and able to manage competing priorities
- Good judgement
- Able to take and follow through with delegated tasks and accountability
Requires sitting or standing for long periods of time, working in an office and clinical environment. Able to stoop, kneel, bend at the waist, and reach on a daily basis. Able to lift and move up to 25 pounds occasionally. Working under stress and use of the telephone required. Manual dexterity required for use of calculator and computer keyboard and other office machines. Hearing and vision corrected to normal range. Must be a team player and possess excellent customer service skills.

Pay: $15.00 - $18.00 per hour
