Job description
Position Type/Expected Hours of Work: Per Diem - Monday - Friday; flexible to the needs of the program. Could include weekend hours if requested by family.
Qualifications: Special Education degree (Bachelors and/or Masters) and appropriate New York State certification (initial or professional) as a Special Education Teacher. Must possess Birth Grade 2 certification to provide Early Intervention Services per NYS DOH regulations. OR can possess a valid NYS license required for Speech Pathologist, Physical Therapist or Occupational Therapist.
Work Authorization/Security Clearance: Authorized to work in the United States and pass all background clearances.
Summary/Objective: Responsible for completion of Early Intervention Multidisciplinary Evaluations as assigned. Will carry out duties in accordance with the regulations of the New York State Department of Health as it pertains to the delivery of Early Intervention Services.

Complete Early Intervention Multidisciplinary Evaluations as assigned.- Successful completion of assessment using standardized testing in all requested domains which could include all of the following: Social History, Cognitive, Social-Emotional, Adaptive, Physical and Communication domain.
- Written report completed and submitted by due date to meet NYS DOH regulations. Written report must include recommendations for direct service delivery if child is found eligible and written recommendation for the family to support the child in the home during daily routines.
- Occasional participation in initial IFSP meeting, if requested.

Travel: All evaluations must be completed in the childs natural environment (home or daycare) as requested by the family.
Work Environment: Use of items to complete Early Intervention Evaluations, to include toys, puzzles and other various manipulative. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand, walk, lift, kneel, bend, twist, use hands to finger, handle or feel; and reach with hands and arms. Work may require lifting, pushing, or pulling in excess of 30 pounds on a regular basis when assisting with the children.
