A Family Connection Consultant (FCC) works with families in Maricopa County referred by the Department of Child Safety to participate in the Family Connections Program. An FCC receives in-depth training from DCS to help families identify areas of need and empower them towards solutions. As an FCC, you will provide personalized services to best meet the family's needs as identified with the program assessments. FCC’s meet families once or twice a week at a time that is convenient for them to help them reach their established goals, including but not limited to locating housing, learning about available community resources, learning new coping skills, enrolling in counseling, or applying for Title XIX or Title XXI services. FCC’s works in collaboration with DCS Specialists and any other agency providing families with support. A full-time FCC is expected to serve 6-8 families at a time while completing required documentation of the meetings.
Summary of Essential Job Functions
· Implementation of the program in accordance with contract requirements and DCS Standards of Practice.
· Family Connection Consultant will conduct home visits assigned in Maricopa County and maintain a caseload of 6-8 families for full-time staff
· Complete home visits weekly or twice a week and help families achieve their UBSMART goals.
· Frequency of contact is determined by DCS via the referral of the family.
· Prepare for home visits and complete documentation of visit within 48hrs of completion of the visit.
· Conduct home visits at a time that works for the family. This may include evenings and weekends if that is the only time the family can meet.
· Complete all documentation within two business days (48hrs).
· Maintain case records and data system in accordance with agency and program policies, procedures, and requirements.
· Meet the requirements of being a court-mandated reporter, reporting incidents and/or concerns of child abuse within stipulated timeframes.
· Initiate contact with referred families within 24hrs of referral and actively work to engage families in the program.
· Travel to and from home visits for referred families in Maricopa County, program trainings and/or events using personal vehicle.
· Attend mandated virtual or in-person trainings.
· Attend meetings with DCS Specialists and/or other providers as required.
· Connect families to resources in the community that address their needs.
· Maintain a positive working relationship with coworkers, DCS Specialists, collaborative partners, and others within the community.
· Participate monthly in two hours of individual in-person or virtual supervision and at a minimum one hour of team meetings.
· Attend and participate in all mandatory conferences, seminars, in-service training, and program meetings.
· Build and maintain relationships with community agencies and partners to share program information and learn about resources available for families (events/outreach as assigned).
· Follow agency dress code and overall agency policies including when delivering services.
· Follow all health safety policies and procedures.
· Maintain a professional demeanor at all times.
· Identify and develop professional goals with FCC Supervisor.
· Other duties as assigned by FCC Supervisor
Requirements
· Minimum of a bachelor’s degree in Social Work, Family Studies, Education, Early Childhood Education, or related field.
· Have knowledge/experience working with children/families
· Attend 4-day training conducted by DCS in Phoenix, AZ
· Bilingual (English/Spanish/Other) skills preferred.
· Have working knowledge of community resources.
· Experience working with culturally and linguistically diverse populations.
· Ability to communicate effectively, both orally and in writing, with families, social service providers, collaborative partners, co-workers, and FCC Supervisor.
· Ability to work flexible hours including evenings and some weekends.
· Ability to work effectively as part of a team.
· Comply with all agency and program policies and procedures including confidentiality, employee conduct, computer usage, and dress code.
· Attend required agency and program/department meetings.
· Adhere to the schedule agreed upon with the supervisor.
· Follow and model AGAF’s core values.
· Must be eligible and able to obtain a Level One-Fingerprint Clearance Card.
· Must have a valid Arizona driver’s license, vehicle and meet agency liability insurance requirements
Ability Requirements
· Sitting for extended periods daily; standing and walking short distances.
· Moderate twisting, bending, stooping, and lifting up to 40 pounds in the performance of assigned duties.
· Ability to use a variety of office equipment and machines as referenced.
· Normal manual dexterity and eye-hand coordination required.
· Repeated hand-wrist movement required.
The nature of the position involves a flexible working environment with multitasking of many different duties and tasks. General daily priorities may change at a moment’s notice and the position requires quick response time and flexibility.
