Do you love coordinating and scheduling appointments and you are interested in the healthcare field? If so, CTF Illinois has the perfect position for you as a Healthcare Coordinator around the Chicagoland Suburbs! At CTF Illinois, we provide services for adults with developmental disabilities and the objective of this position would be to coordinate and schedule medical services and appointments for our individuals.
Essential Job Functions:

  • Check monthly for appointments (i.e. medical, vision, dental, neurological, etc.) that are due to be scheduled for residents.
  • Schedule and confirm appointments, coordinate transportation needs, and accompany individuals on all appointments.
  • Follow-up as necessary and appropriate.
  • Assist nursing to check in monthly medications
  • Ensure that a medical synopsis form (completed by the nurse) is taken to the appointments as appropriate, returning all appropriate documentation to nursing for follow-up.
  • Ensure physician orders are signed by physician monthly and send a copy of physician orders to the pharmacy each month
  • Distribute Medical Administration Record (MARS) and Physician Orders appropriately each month.
  • Complete quarterly medication reviews and ensure physician signature is obtained.
  • Assure the delivery of each individual's financial assistance for the payment of cost of care, food stamp eligibility, and medical services by coordinating benefits with the Illinois Department of Human Services.
  • Ensure all individuals have medicaid cards.
  • Adhere to terms and conditions of employment as outlined by Employee Handbook and job description.

Work Schedule: *

  • Work hours should coincide with Day Program operation hours, which are typically Monday – Thursday, 8 AM – 5 PM, Friday 8 AM – 4 PM, but may be subject to change, per supervisor request.

Required Experience, Education and/or Training:

  • High School Diploma or general education degree (GED).
  • Basic user knowledge of Microsoft Office (Excel, Word, Outlook,etc.), and/or other PC applications.

Additional Experience, Education and/or Training Preferred: *

  • Prefer one year working experience directly with individuals with developmental disabilities.
  • Previous DSP certification preferred.

Job Type: Full-time

Pay: $15.00 per hour
