What We’re Looking For
Are you looking to better your community? Are you ready to get involved? We have a career opening for a Health Specialist to join our Neighborhood Connections Team. Trillium’s Health Specialist role has been created to take part in grassroots solutions for community development. The Health Specialist will participate in efforts to address food insecurity, employment and education challenges, community inclusion and social isolation, interpersonal violence and toxic stress, transportation barriers, and we work in collaboration with the Housing Department to address housing-related challenges.
On a typical day, you might:
- Receive referrals related to members’ unmet health related resource needs including, addressing the following:
- Healthy eating and food insecurity
- Loneliness and isolation through inclusion
- Education and employment challenges and income access
- Interpersonal Violence and Toxic Stress
- Transportation barriers
- Housing needs
- Represent Trillium Health Resources by involvement in regional activities, community programs and inter-agency conference and meetings.
- Cultivate and maintain effective relationships with agency personnel, county and state officials, community organizations and the general public.
- Assist with development and coordination of projects and programs ranging in size from a single county or to region-wide change initiatives.
- Offer member orientation, developing, presenting and teaching a variety of workshops to members in a variety of home and community based settings on topics such as health and wellness concepts and how to access community resources.
- Utilize designated software platforms to make warm hand-offs with community based organizations and track outcomes for referrals.

Qualifications:
Education: A High School Graduate or GED is required.
Experience: A minimum of two (2) years’ experience in the Health and Human Services field, including experience working with any combination of the BH/SU/IDD population and/or experience with public speaking/training and/or teaching or equivalent of higher education or higher education/experience.
Prefer lived experience, with Community Development experience, and/or experience with community based organizations and/or community partners centered around housing, food insecurity, transportation, education & employment, interpersonal violence/toxic stress and/or social integration and inclusion.
License/Certification: None Identified.
Must have a valid driver’s license
Must reside in NC and must live in the region they support.
