Borinquen Medical Centers is located in Miami, FL and is seeking to hire a full-time Health Nurse Educator to join our team! The Health Promotion & Education Nurse (HPEN) will aid in the planning, implementing, and developing of curricula for educating patients. It is crucial to include healthcare topics related to chronic conditions to improve patient care and self-efficacy, utilizing scientific data and experience to educate patients and their caregivers/family members. The HPEN is an integral part of the Patient Care Team following a patient centered approach. The HPEN will work with populations identified or qualifying for group visits to address and improve health disparities including but not limited to chronic conditions. The HPEN will implement, coordinate, and support various group visit programs as deemed necessary by the Chief Medical Officer to improve patient involvement and self-care as well as to improve productivity and schedule utilization by department/specialty.
TASKS AND RESPONSIBILITIES
Core duties and responsibilities include following and implementing access to care for patients whose diseases are included in the QI Plan yearly as well as other quality measures needed to be addressed, which nurse education and nursing skills are required working in conjunction with the Q&P team and other clinical staff.
Coordination of Care includes:
- Attends departmental meetings
- Evaluates in general with Director of Ancillary Nursing and Chief Medical Officer the overall population health and provides and implements disease management strategies, including care gap evaluation and provider engagement to fulfill and improve outcomes.
- Supports and aides daily group huddles within departments, helping validate nursing staff performance including the engagement of the patient care team and the clinical manager.
- Supports patient and family members to accomplish goals in a patient centered environment.
- Coordinates with the Quality Department to receive and review lists of patients identified as being able to benefit from Shared Medical Appointments and/or Group Educational visits
- Implements, supports, and coordinates group visits for OBGYN, Pediatric, Adult Medicine, Specialty and Complementary Medicine departments as needed.
- Supports all Q&P initiatives such as QI QA Plan, Meaningful Use, UDS, HEDIS amongst many other that improve the health of the population and improve their outcomes.
- Supports organization wide initiatives and participates in nurse centered projects or programs such as but not limited to immunizations as well as safety programs such as CPR drills and infection control.
REQUIREMENTS
- LPN with over one year of experience, RN preferred.
- Current or familiar with Certified Diabetes Educator certification (CDE), preferred.
- Degree in nursing and knowledge of disease management and education
- 1-2 years of experience in the healthcare field.
- Well organized
- Bilingual preferred.
- Strong interpersonal skills.
