The Health Team Lead provides comprehensive health services to people experiencing homelessness or who are low-income within the San Fernando Valley area. The specialist will be responsible for providing crisis intervention, assessing health needs, and working collaboratively with other programs within the agency. The Health Specialist will be accountable for providing field-based services, making follow-up calls and visits, making warm handoffs, and making connections with local health resources.

Essential Duties:

  • Provide street-based outreach and clinical case management to people experiencing homelessness and low income within the San Fernando Valley.
  • Advising on crisis interventions and working within the outreach team to help deescalate situations.
  • Support the agency by providing health workshops to community members, and staff as needed.
  • Ensure that persons with health problems are identified; work with the individuals to develop a health service plan, if desired.
  • Create relationships with health service providers and develop the referral process; maintain a resource listing of health services available within the San Fernando Valley
  • Utilize evidence-based practices such as motivational interviewing, harm reduction, and housing first models.
  • Complete follow-up with clients to ensure that referrals are completed and provide advocacy on their behalf if needed.
  • Increase engagement of participants and/or increase awareness surrounding health symptoms.
  • Support MSW interns with guidance and direction at the agency.
  • Participates in agency meetings and community meetings as designated by the supervisor
  • Additional tasks, projects, and responsibilities as assigned by the supervisor

Qualifications:

  • Master’s degree in Social Work (MSW/ACSW/ASW), Marriage Family Therapist (MFT/AMFT), Counseling Psychology, or similar is strongly preferred
  • 1 year of experience providing health case management services
  • Ability to demonstrate empathy and a non-judgmental attitude when engaging with persons experiencing homelessness
  • Experience with HMIS system and reporting is a plus and must be willing to learn new systems for data tracking
  • Possess a high level of tolerance and understanding for individuals who present for services with health and physical health needs
  • Bilingual: Spanish/English preferred
  • Must have a valid driver’s license and be vaccinated against COVID-19.
  • Strong experience working within team environments and ability to collaborate

Job Type: Full-time

Pay: $22.00 - $25.00 per hour
