Lakeview Center provides comprehensive behavioral health care to adults and children with mental illnesses, drug and alcohol dependencies and intellectual disabilities. Across Northwest Florida, our services range from residential treatment to outpatient counseling, psychiatry, trauma care, treatments for substance misuse and 24/7 support for those with serious mental illnesses. Healthy Start Program Provides education and support during a woman’s pregnancy, developmental screenings for children ages 0-3 years old, and makes referrals to help meet the needs of the women and children. The goal of treatment is to have a healthy pregnancy and healthy child development. Services are provided in home, through telehealth, or telephonically.

Overview:

The Healthy Start Care Coordinator provides care coordination, case management, education, psycho-social counseling and referral/follow-up to women during pregnancy and to their infants after delivery and up to the age of three years. The coordinator position also provides therapeutic services to clients within scope of training, protocols, and competence, and within requirements of statutes applicable to this position. Therapeutic services include but are not limited to: assessment, service planning, direct service provision, person centered services and management of client risk and safety. The Healthy Start Care Coordinator exercises independent judgment and works with minimum supervision. Work may be performed in a variety of settings including office and client’s home.

Qualifications:

• Bachelor’s degree in one of the following areas: a) Social sciences b) A health-related field such as nursing, health education, health planning, or health care administration c) Social work or related field
• .OR Two (2) years of college with three (3) years of public health/maternal-child health experience. • Proficiency in computer applications; detailed orientated, quick learner, self-motivated, and deadline oriented; Experience working with diverse communities, organizations and agencies
• .Bi-lingual preferred
• .Must be able to pass organization screening requirements, including state or federal background screenings as appropriate
• .Must possess a valid driver’s license from the state in which you reside, a good driving record and be insurable under the corporate policy.
