Job description

The Healthy Start Care Coordinator works to provide professional, medical and social care coordination to children, adults, fathers, and families within the Healthy Start Initiative; provides short-term counseling to clients to resolve problems; and refers to other specialized agencies for additional assistance or treatment.

Duties and respoinsibilities include but are not limited to the following; Provide casework services and determine perferred intervention strategy for each cient according to Healthy Start protocols. Complete and maintain a database. Interview clients' families; complete intake and risk assessments which identify specific needs of each client and family. Manage a case load of 40-55 clients. Follow up on appointments, make home visits. Work with the Liaison to ensure appropriate services are being offered to maternal particpants of the program. Recruit and encourage male participation in the fatherhood program.

Responsible for all written and oral communication with participants and recruitment activity in the community. Perform other duties as assigned.
