Job description

We are looking for a Physical Therapist to join our team and provide home health services to our clients. The ideal candidate will have experience in providing physical therapy services in a home health setting, and will be able to assess, plan, implement and evaluate individualized treatment plans for each patient. Our Physical Therapist will also be responsible for providing education and support to patients and their families.

Responsibilities:

• Evaluate patient's physical condition and develop a plan of care.
• Provide home health physical therapy services to patients in their homes.
• Instruct patients in proper use of therapeutic exercises, activities, and assistive devices.
• Assess patient's response to treatment and modify treatment plans as needed.
• Educate patients and families on how to prevent or manage their condition.
• Document patient's progress and communicate with other healthcare professionals.

Qualifications:

State licensure as a Physical Therapist

Open to all experience levels, including new grads

*Able to work full-time or part-time hours including 2-3 weekdays of availability

*Comfortable seeing patients in their homes

*For PRN, some experience is required
