Overview:
KORT is Kentucky's premier provider of outpatient physical and occupational therapy specializing in orthopedics, sports and industrial medicine. Since 1987, KORT has provided clinical and customer service excellence to patients in our clinics, at employer sites, and with high school, college, and professional sports.
In addition, KORT Home Health also contracts their home health services in Kentucky including: post-operative physical therapy; fall prevention; vestibular rehabilitation; wound care; and more.
Compensation starting at $50,000 annual base plus mileage reimbursement.
Select Physical Therapy is part of Select Medical's Outpatient Division. We are a nationally prominent, locally driven provider of outpatient physical rehabilitation with approximately 1800 locations in 39 states, including the District of Columbia. Throughout the country we operate primarily as Select Physical Therapy, NovaCare Rehabilitation, Physio, Kessler Rehabilitation Center, KORT, Saco Bay Physical Therapy, Concentra, SSM Physical Therapy and Keystone Physical Therapy. Our highly respected clinical team provides preventative and rehabilitative services that maximize functionality and promote well-being. We develop individualized treatment plans to help achieve each patient's specific goals. Our integrated local market network allows us to effectively partner with physicians, employers, payors and case managers to achieve optimal patient outcomes in a cost-effective manner. We also provide physical therapy and athletic training services to professional sports teams, colleges, universities and high schools. #ACN Responsibilities:
POSITION RESONSIBLITIES
1. Perform the initial home care patient visit (if indicated) and re-evaluate the patient’s needs and progress on a regular basis. *
2. Initiate the plan of care (if indicated) under doctor’s orders. *
3. Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patient. *
4. Observe, assess, and document symptoms. *
5. Monitor reactions and patient progress. *
6. Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans. *
7. Educate patients and caregiver on techniques for in home health care. *
8. Coordinate patient services. *
9. Supervise LVNs and HHA’s. *
10. Notify the physician and other personnel (Clinical Supervisor, PT, Case Manager) of change in the patient’s condition. *
11. Perform skill outline in the agency’s approved policy and procedure manual. *
12. Discharge the patient from skilled nursing services when the discharge criteria have been met. *
13. Case conference clinicians providing care to ensure coordination of care. *
14. Update clinical records according to policy and procedures. *
15. Update knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually. *
16. Provide onsite supervision of LVN/HHA. *
17. Adhere to Medicaid and Medicare regulations.
Qualifications:
The Registered Nurse position requires:
Be a registered nurse (R.N.) with a current Indiana license.
Minimum of one year of nursing experience as a R.N.
One (1) year of Home Health experience preferred.
Work positively and favorably with patients, families, and staff.
Strong verbal and written communications skills
Flexible with diverse client assignments.
Current automobile liability insurance and current unrestricted drivers license.

Provides care consistent with all relevant professional standards and state and federal requirements. Demonstrate proficient skills using Oasis to admit, transfer, re-certify, and discharge home health patients in regards to physician orders. Coordinate and supervise LVN’ s and HHA’s in the delivery of patient care. Maintain compliance with agency policy and procedures. Follow Medicare and Medicaid regulations.
