Cypress at Home is seeking a full-time Scheduler to join our Home Health team!

Our home health scheduler plays a very important role within the company – serving as the front-line liaison between the agency, clients/residents, and employees.

Position may include evenings and weekends. Compassion for the elderly is a must!

WHAT WE ARE LOOKING FOR:

• Home Health scheduling experience - Medicare familiarity a must!
• Knowledge of and ability to ensure compliance with the statutory and administrative rules of the state, federal and CHAP guidelines for home health agencies.
• Able to demonstrate strong organizational and problem-solving skills and deal effectively with multiple tasks.
• Able to communicate with clinical and non-clinical personnel and clients/residents.
• Able to work as a team player as well as work independently.
• Able to quickly learn new systems including Home Health specific software.

WHAT YOU WILL BE DOING:

• Assists in the coordination of visit schedule for all staff.
• Assists in the reconciliation of schedules with clinical itinerary records.
• Scheduling of all clinical staff for the agency in accordance with resident/patient needs and DON requests Plotting Disciplines and Frequencies in Patients Chart (SN/PT/OT/SL/MSW/HH)
• Scheduling Clinicians to Patients.
• Initiating and Tracking Consents.
• Files documents using a pre-established, routine filing system.
• Maintains confidentiality of patient, employee and agency matters.
• Performs clerical duties including typing memos, correspondence, emails and/or responding to Teams messages as needed.
• Data entry of documentation accurately and timely.
• Maintaining EMR data as needed for scheduling and documentation.
